Patient Policy Agreement

Our optometric physicians are highly skilled practitioners who will be working closely with student doctors to provide you with quality eye care using state of the art equipment. In order to maintain an optimum level of visual and ocular health, we will need to work together as partners in caring for your eyes. By understanding and agreeing to comply with our policies, you will enable us to provide you with the kind of care you deserve.

  • An annual comprehensive eye examination is a prerequisite to a contact lens fitting. Patients may have an examination at the Eye Institute or provide a copy or summary of the medical records from the examining eye care provider. 
  • The cost of the professional fees varies by the complexity of the contact lens fitting and includes a lens care starter kit and routine contact lens related follow-up care for one year.
  • Professional fees do not include the cost of lens materials, with the exception of bandage contact lenses and orthokeratology.
  • A written contact lens prescription will be provided upon the completion of the fitting process, which may include multiple visits.
